A Return to Work Release note is required to be provided before the start of your first shift back at work. This note should be written by your physician or healthcare provider and detail any work restrictions upon your return, if applicable.
Your Return to Work Release note should be faxed, emailed, or dropped off in person right away upon receipt to your local HRBP (listed under your Associate profile in myCareer@Hollister). Failure to provide this note means that you will not be allowed to return to work, and any additional time away will not be protected and instead will be subject to Hollister's standard attendance policy.
When planning for your first day back at work, be sure to confirm with your local HRBP that your Return to Work Release note was received. |
